The situation

What has happened

On 8 September 2026 a horse reared while being loaded into float bay 3 at Cranbourne. The handler stepped back into the path of a second horse being led past. Nobody was hurt, but the Stable Foreman reported it the same day and the People, Safety and Compliance Manager has assigned you the investigation. A partition pin incident in August already changed the pre-departure checklist, and a rider fall on 1 September is still under investigation. The fortnightly toolbox talk is on 15 September and the manager wants the findings presented there with agreed actions.

Your brief. Investigate the near miss using the Incident and Hazard Register entry, the procedure and interviews, identify contributing factors and corrective actions, update the register, and prepare the toolbox talk record for 15 September.
