Horse Treatment Record Form

Purpose. This form is the paper record of a treatment given when the Treatment and Medication Log cannot be reached, for example at a race meeting or in a paddock, and must be transferred to the log the same day.
1.When to use this form
Use this form only when you cannot enter the treatment in the Treatment and Medication Log before treating the horse. The form must be entered in the log by the person who treated the horse, or their supervisor, before the end of the day.
2.Completing the form
Fill every field. If a field does not apply, write not applicable. The product name must be the full name on the label, and the dose must be in the units on the label. Record the withholding period given by the veterinarian or label.
3.Authorisation
Prescription medications must show the name of the authorising veterinarian. Supplements and over-the-counter products must be on the approved list. If in doubt, phone the Veterinary Liaison and Welfare Manager before treating.
4.Race day
No treatment may be recorded on race day for a runner except where the rules permit and the stewards have been advised. Check with the Racing Manager before treating any horse on a race day.
5.Filing
Once the treatment is entered in the log, write the log entry number on the form and file it in the treatment folder at the property office. Forms are kept for two years.
